Job Description: Position Summary: Funded under a grant from the Dept of Public Health, the Compass Moms Do Care Program, part of the Maternal Newborn ServicesTeam, was created to address the complex social and medical needs of substance-exposed neonates and their mothers. The outpatient Compass Moms Do Care program will provide prenatal care and substance use treatment to pregnant women during pregnancy and up to 3 years post-partum as well as women parenting a child up to three years old. A Peer Recovery Mom is a support paraprofessional and part of an integrated program who promotes recovery to women with substance use disorder (SUD) by encouraging, mentoring, educating and linking women with SUD to the resources in the recovery community. Reports to the Program Manager. Essential Role Responsibilities u00b7 Provide supportive coaching via home visits, program group meetings, hospital visits or phone/text messaging at scheduled intervals determined by the need of the client u00b7 Be knowledgeable about the substance use disorder service system and link the individual to treatment and recovery resources including but not limited to Acute Treatment Services, sober housing, benefits, and Narcan education. u00b7 Act as an advocate for the needs of the individual u00b7 Demonstrate an understanding and belief in the recovery process and Recovery Oriented Systems of Care Model u00b7 Knowledge of basic crisis intervention, Motivational interviewing, and case management techniques u00b7 Demonstrate good boundaries regarding confidentiality and personal relationships, while at the same time be able to share personal knowledge about recovering from a substance use disorder. u00b7 Demonstrate the ability to evaluate what is needed by each individual and adjust approach accordingly. u00b7 Provide care coordination amongst providers u00b7 Link women to peer network/natural supports when able/appropriate u00b7 Facilitate and coordinate referrals to community social service and medical providers as requested and as needed by clinician or care provider including: assisting clients in accessing further treatment, self-help groups, housing and services related to physical/behavioral health and other activities of daily living u00b7 Connect patient with emergency housing, shelter or residential programs as needed u00b7 Arrange transportation for patient when necessary u00b7 Attend peer recovery, trauma informed care, seeking safety and other applicable training classes u00b7 Attend and assist with gender-specific, trauma-informed therapeutic groups u00b7 Attend weekly program staff meeting, weekly individual supervision and conferences/workshops as appropriate u00b7 Other program related duties as assigned Education/Experience: 18 years or older and high school diploma required; bachelor degree preferred. Recovery Coach Certification plus two years of experience working in the recovery community Driveru2019s license and car required Have lived experience with pregnancy and addiction Have at least two years of current, sustained recovery without relapse Pay Range: $22.00 - $32.00 The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
