Job Description u00b7 Support our patients, families, and loved ones, as they go through their end of life journey u00b7 Perform a comprehensive assessment to establish goals for symptom management and end of life care with effective case management strategies u00b7 Be a member of an active interdisciplinary team that addresses patientsu2019 needs holistically u00b7 Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families. u00b7 Utilize state-of-the-art technology to document skilled services provided. u00b7 Registered Nurse with Valid CT RN license u00b7 One year of RN experience; Homecare or Hospice experience preferred. u00b7 Valid driveru2019s license and reliable transportation required. u00b7 Computer proficiency required.
