Job Opportunity: Registered Nurse (NYIAP) - Brooklyn, NY We are seeking a passionate and experienced Registered Nurse to support the New York Independent Assessor Program (NYIAP). In this role, you will perform vital home assessments for vulnerable populations to determine eligibility for long-term community-based services while mentoring a team of Nurse Assessors. The Basics + Pay Rate: $60/hr + Location: Brooklyn, NY (Regional field travel + Remote/Home Office) + Hours: Monu2013Fri (8amu20136pm) Satu2013Sun (10amu20136pm) _Schedule flexibility available. 37.5 hours weekly_ Key Responsibilities + Clinical Assessments: Conduct initial in-home evaluations (or telehealth) using the UAS-NY tool to determine health status and care needs. + Leadership & Mentorship: Support the Regional Nurse Manager by facilitating training, conducting demonstration visits, and providing feedback to new IA Nurse Evaluators. + Documentation: Review medical records, enter evaluation data electronically, and ensure continuity of care by eliminating gaps in treatment plans. + Collaboration: Maintain relationships with consumers, caregivers, and facility staff; coordinate with the Call Center for plan enrollment. Qualifications + Licensure: Valid RN license in the state of New York. + Experience: 2+ years of RN experience in home care. + Technical Skills: Prior experience using the UAS-NY Community assessment tool (OASIS). + Travel: Ability to travel locally up to 50%. Must have a reliable form of transportation (public transit is acceptable). + Equipment: Must have a personal cell phone. (Company provides computer equipment).
