What will Flexible Scheduling Offer Me? u2022 Extended Hours Monday-Friday starting 7:30a-7p u2022 Extended Hours Saturday & Sunday starting 7:30a-7p u2022 Nurse Assessor Can Build Their Own Schedule Each Week u2022 Nurse Assessor Can Make Up Hours for Interrupted/No Shows u2022 Work Any Calendar Day Pay Rate: $55-$58/hr. _Hourly wage may depend upon experience, education, geographic location and other factors._ Qualifications u2022 New York State RN License, required. u2022 Work experience with UAS-NY or Oasis Community Assessment tool u2022 2+ yearsu2019 experience working in-home setting within the last 15 years, required. u2022 Work experience with the adult/geriatric populations; home care experience, experience working with individuals with physical or developmental disabilities. u2022 Computer literacy: ability to navigate new and emerging technology; ability to perform telehealth assessment; highly developed written and oral communication skills; strong interpersonal skills; and proven ability to work with community groups. u2022 Knowledge of long-term care services and programs u2022 Ability to travel throughout the assigned region, as assigned, including travel to individualsu2019 homes. Highlights u2022 Full county coverage required. u2022 Appointments based on proximity to your home/self-assign your territory. Responsibilities u2022 The incumbent will review the completed Community Health Assessment tool in the NYS Uniform Assessment, to assist the Physician in the completion of a Physicians order. u2022 The incumbent may follow up with individuals who have received an assessment through face-to-face communications. u2022 Responsible for reviewing and making recommendations on consumer assessments for long term care services and supports. u2022 Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes. u2022 Work collaboratively to monitor and address clinical quality and gaps in care. u2022 Provide clinical and educational assistance to support project initiatives. u2022 Demonstrate communication skills necessary to interpret age-specific responses.
