Summary The Women's Health Care Coordinator (WHCC) is responsible for providing competent, evidence-based care to assigned patients. The incumbent of this position provides patient management, care coordination, and disposition planning for Veterans, including but not limited to addressing mammography, cervical cancer screening, and/or bone density testing. Responsibilities The Registered Nurse (RN) - Women's Health Care Coordinator (WHCC) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The person in this role assesses, plans, implements, and evaluates care based on age specific components. The RN - WHCC assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. They follow procedures per established policies and guidelines and influence care outcomes by collaborating with members of the interdisciplinary team. The RN - WHCC is also responsible for the following duties: Assisting Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Coordinating care with multiple providers across all levels and sites of care, including assuring services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to Veterans. Providing oversight of the patient's journey through the diagnostic and treatment continuum, ensuring that all steps in the process are scheduled, timely and completed, while tracking the plan of care. Coordinating and facilitating, as needed, WH care including contacting the Veteran. Facilitating appointments and authorizations, tracking receipt of test results and other medical documentation, and notifying providers of needed follow up care. Anticipating and implementing preventive patient care measures. Providing guidance and education to patients and caregivers; collaborating with other professionals at all levels. Addressing psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation with the interdisciplinary care team. Ensuring the plan of care and educational requirements are individualized and tailored specifically to support each individual patient's needs. Demonstrating leadership in delivering and improving WH care through collaborative strategies with others. Other duties, as assigned.
