Our holistic approach includes OBAT, IOP, and mobile treatment units, with a strong focus on patient-centered, evidence-based recovery and community partnerships.Your Role:The Health Homes Coordinator oversees daily nursing operations, manages case loads, ensures compliance with state and federal regulations, supervises staff, and supports quality patient care in an outpatient opioid treatment program (OTP).Key Responsibilities:Supervising case managersManage case loads and ensure high-quality, coordinated patient careLead quality assurance and compliance activitiesOversee medical record documentationCollaborate with internal teams and external community partnersEngage in community outreach and resource coordinationCompensation for roles at Woonsocket CTC varies depending on a wide array of factors, including but not limited to the specific location, role, skill set, and level of experience. Woonsocket CTC provides a reasonable hourly range of compensation for roles that may be hired in Rhode Island, as set forth below.Role Location: Rhode IslandCalculated Salary Range for the role: $77,000.00 - $87,000.00/yr.Qualifications:Active RN license in Rhode Island OR Master’s Degree professional in a relevant fieldMust have experience in Case Management, managing caseloads, and supervisory leadershipExperience in community outreach and connecting patients to local resources2+ years of nursing experience (behavioral health preferred)1+ year of supervisory or case management experienceNo history of substance misuse within the past 3 yearsPreferred Experience:Medication-Assisted Treatment (methadone, buprenorphine, naltrexone)Behavioral health or addiction recovery settings (outpatient, residential, correctional)Knowledge of trauma-informed care, harm reduction, and regulatory compliance (DEA, HIPAA, Joint Commission)Experienced with clinical documentation and billing processesWe are committed to providing equal employment opportunities to all applicants for employment regardless of an individual’s characteristics protected by applicable state, federal, and local laws.#LI-CTCAHCTC
