Working collaboratively with all healthcare team members, you will develop and implement treatment plans that support patient-centered care for individual patients and the medical community.Being dedicated to patients and family-centered care, you value the personal self-determination of your patients. To succeed in this role, you must manage multiple priorities simultaneously and engage in creative, compassionate, and ethical problem-solving.Responsibilities:Perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populationsEstablish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families.Serve on hospital committees and develop and present in-services to Hospital staff.Dignity Health now offers an Education Benefit program for benefit-eligible employees after 180 days.
