Summary The Minimum Data Set Registered Nurse in the Community Living Center is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The RN assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care, administers medications and procedures, and influences care outcomes by collaborating with members of the interdisciplinary team. Responsibilities The Community Living Center Minimum Data Set (MDS) Registered Nurse is responsible for providing competent, evidence-based care to assigned residents and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. Responsibilities include, but are not limited to: Assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others Responsible for coding the sections of the Resident Assessment Instrument (RAI)/MDS Assessment and the Care Area Assessments (CAAs) that are assigned to nursing, adhering to current Centers for Medicare and Medicaid Services (CMS) and Veterans Health Administration (VHA) timeframes of assessment completion. Responsible for the completion of assigned nursing sections in the RAI/MDS Assessment. Participates in the CLC Interdisciplinary Team care plan meetings and reviewing, communication, and monitoring the residents' care plan. Collaborates with the CLC Resident Assessment Coordinator (RAC) to educate CLC staff on the completion of the RAI/MDS process, which includes but is not limited to documentation requirements for accurate MOS data entry, educational sessions pertaining to RAI/MDS software and program updates as needed. Supports quality improvement activities based on CLC quality management data. Participates in ongoing survey readiness activities in the CLC. Documents and taking corrective action when notified by the CLC management team, charge nurse, or designee of any discrepansies in coding or documentation of RAI/MDS assessments and CAAs reported during the interrater reliability review. Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Supports colleagues through knowledge sharing to provide safe, quality nursing care. Questions clinical practices for the purpose of providing evidence-based care.
