Occupational Therapist Pool/PRN Responsibilities: + Use standardized assessments, clinical observations, and patient interviews to determine the impact of the patient's condition on their ability to perform daily tasks. + Identify patient strengths, challenges, and potential goals for therapy. + Collaborate with other healthcare professionals to develop individualized treatment plans based on patient assessment results. + Design and implement interventions aimed at improving patientsu2019 functional abilities and independence in daily activities. + Provide hands-on interventions to improve fine motor skills, gross motor skills, strength, coordination, and cognitive function. + Implement techniques to improve memory, attention, problem-solving, and other cognitive functions, especially for patients with neurological conditions. + Support family members and caregivers with understanding the patientu2019s needs + Maintain detailed, accurate, and timely documentation of patient progress, treatment plans, and outcomes in accordance with hospital policies and regulatory requirements. + Evaluate a patientu2019s readiness for discharge, ensuring they are safe and able to perform necessary ADLs independently or with the help of family and caregivers. + Provide recommendations for home modifications, further rehabilitation, and outpatient therapy if necessary. + Ensure a smooth transition from inpatient care to outpatient or home-based therapy services.
