Overview: We are seeking a dedicated and detail-oriented Medication Reconciliation Pharmacist to join our team at the Virtual Care Center. The ideal candidate will be responsible for training Medication Historian Technicians, verifying submitted medication lists, participating in annual medication reconciliation Leapfrog audits, building strong relationships with the inpatient pharmacy team, and ensuring seamless integration within the virtual care environment. What to expect: Training Medication Historian Technicians: Provide ongoing education and support to ensure accurate and efficient medication history collection. Provide monthly education on various topics including high-risk drugs to ensure technicians are well-versed in identifying these medications. Checking Submitted Medication Lists: Review and verify medication lists submitted by Medication Historian Technicians for accuracy and completeness. Ensure discrepancies are addressed with the provider and resolved promptly. Participating in Annual Medication Reconciliation Leapfrog Audit: Actively participate in the annual Leapfrog audit process. Collaborate with audit teams to ensure compliance with medication reconciliation standards. Building Relationships: Foster strong working relationships with inpatient pharmacy staff, providers, and MHTs . Collaborate on medication management strategies and patient care initiatives. Working in the Virtual Care Center: Integrate medication reconciliation processes within the virtual care setting. Utilize telepharmacy tools and technologies to support patient care remotely. Requirements: PharmD degree from an accredited institution. Active pharmacist license in the state of practice. Experience in medication reconciliation preferred.
