As part of our interdisciplinary Hospice team, ourCNA's (Home Health Aides) provide patients with care in the comfort oftheir own home and/or similar setting. We serve patients throughoutMilwaukee, Waukesha, Ozaukee, and Washington Counties (mileagereimbursed between visits). As aHome Health Aidein Hospice you willprovide direct assistance with activities of personal care, maintenanceof patient health, Agency facilitation of treatment for patients, andcontribution to a health environment for patients. These activities areprovided under the direction and supervision of a Registered Nurse orTherapist, in accordance with the physicians' orders. Support patientcare by reviewing care plan in the home prior to providing cares.NotifyCase Manager with issues / concerns regarding care plan and when caresneed revision. Provide assistance with daily living under thesupervision of the Case Manager i.e., bathing, dressing, transfers,range of motion, toileting, etc. Explain to patient the care beingprovided and involves patient in cares. Consistently complete mobilecare upon arrival and departure from patients' home per Agencypolicy.Mobile Care includes tasks completed in accordance with the plete mileage logs timely and accurately. Work cooperativelywith patients, care providers and family members, as well as withappropriate Agency staff by listening and responding to patient needs,engaging in appropriate communication, reporting pertinent patientinformation to Case Manager in a timely manner Regularly complete homemaintenance per plan of care i.e., house cleaning, washing dishes,laundry, meal preparation, and transportation, etc. Horizon OffersOurHome Health Aides: Guaranteed Hours-Full-time opportunities availableCompetitive Pay -Plus, Mileage...
