OverviewNavitus Health Solutions is seeking a Pharmacist, Telehealth to join ourteam The Pharmacist, Telehealth will be a key contributor to thesuccess of a newly launched Clinical Engagement Center (CEC) focused onimproving member's health and wellness via tele-pharmacy and wellnesscoaching. This pharmacist is responsible for the delivery of theMedication Therapy Management (MTM) services for commercial and Medicaremembers. In addition, the Telehealth Pharmacist will assist in thedevelopment and execution of additional clinical outreach program toexceed client expectations. Is this you? Conduct medicationtherapy management (MTM) and expanded clinical programs in accordancewith the Centers for Medicare & Medicaid Services (CMS). Developappropriate clinical algorithms, pathways, and call scripts to supportpharmacy staff in delivering MTM services. Review and update allalgorithms and call scripts at least yearly and with updated clinicalguidelines.
