Responsibilities include: + Completing start-of-care and routine nursing visits + Assessing patient condition and developing care plans + Providing medication and disease-management education + Monitoring progress and preventing hospital readmissions + Coordinating care with physicians, therapists, and care team members + Identifying changes in condition and communicating appropriately + Documenting visits in the electronic medical record + Maintaining HIPAA and patient safety standards _Training & Support_ You will not be sent out unprepared.
