Responsibilities include: + Completing routine skilled nursing visits + Performing patient assessments + Providing medication and disease-process education + Monitoring wounds, vitals, and overall health status + Communicating changes in condition to providers + Coordinating care with therapists and care team members + Documenting visits in the electronic medical record + Following HIPAA and patient safety standards --------------------------------------------------------------------- Training & Support You wonu2019t be sent out alone. We provide: + Structured onboarding + Ride-along visits with experienced clinicians + EMR documentation training + Ongoing clinical support and case consultation --------------------------------------------------------------------- Requirements + Active, unrestricted RN license + At least 1 year of nursing experience in any setting (hospital, SNF, clinic, rehab, etc.) + Reliable transportation, valid driveru2019s license, and auto insurance + Ability to travel within the Lafayette coverage area + Comfort working independently in the field + Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs. Preferred (not required): + Home Health or Hospice experience + HomeCare HomeBase (HCHB) experience + ASN or BSN --------------------------------------------------------------------- _Important Information_ This is a PRN/per diem position and is not benefit-eligible (no health insurance, PTO, or 401k).
