DescriptionCommunity Outreach Mission & Overview:This is a highly autonomous, project-based Community Outreach role focused on expanding HHVI's mission throughout the community and reaching populations we have not previously served. The position supports existing and/or future outreach efforts with UH while also independently planning, organizing, and executing community-based events.The Outreach RN engages directly with community members and patients through health education and screenings-e.g. commonly blood pressure screenings-and connects individuals to hospital services as needed by coordinating appropriate referrals and follow-up care.The role typically involves coordinating and staffing a few outreach events per week, including planning and leading three annual signature events (such as Go Red, STEM initiatives, and Heart Heroes), as well as participating in large-scale community partnerships (e.g., UH Health & Safety Expo, Cleveland Browns events, and other regional activities).This position requires strong initiative and leadership-someone who can
