Details Location: Mequon, WI Facility: Sarcred Heart Rehab Department: Acute Rehab Schedule: Day shift, 20hrs/wk, Monday-Friday hours are 8am to 4:30pm with some alternating week-end time. Travel is involved. Your recruiter will provide the most up-to-date details during the hiring process._ Responsibilities + Referral and Admissions: Act as the primary link between internal and external referral sources and inpatient rehabilitation operations. Drive patient admission volume by establishing relationships and facilitating communication with key stakeholders, including case managers, physicians, payors, patients, families, and various referral sources. + Clinical Evaluation: Ensure appropriate patient selection for the inpatient rehab unit. Conduct clinical chart assessments during both on-site and off-site patient evaluations, working in close collaboration with the physician. + Education and Outreach: Serve as an educational resource for patients, family members, and caregivers. Provide in-service training and educational opportunities for physicians, case managers, interdisciplinary team members, and other healthcare professionals. + Business Development: Contribute to the development and maintenance of promotional materials for referral sources. Participate in the creation and execution of business plans for the rehabilitation unit. Requirements Licensure / Certification / Registration: + One or more of the following required: + Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date. + Rehab Therapist specializing in Occupational Therapy (OTR) credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date. + Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date. + Speech-Language Pathologist credentialed from the Wisconsin Hearing and Speech Examining Board obtained prior to hire date or job transfer date. + BLS Provider preferred. American Heart Association or American Red Cross accepted. + Drivers License preferred. Education: + High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required. Additional Preferences Previous experience in long term care/inpatient rehab or acute rehab setting. Ability to be self directed, prioritize patients to be seen, comfortable with electronic health record navigation. Excellent verbal and written communication. Ability to assess the patient and communicate to a physician if the patient is appropriate for an inpatient rehab setting.
