FTE: 1.000000 Standard Hours: 40.00 Shift: 1 ShiftDetails: This position will primarily be first shift with hours between6am and 6pm Monday through Friday. Some flexibility is required, withoccasional early mornings or later evenings based on departmental needs.On-Call Rotation: The individual will have on-call responsibilities on arotating basis, including weekends and holidays. The rotation currentlyalternates every 4 weeks between Friday-Sunday coverage andMonday-Thursday coverage. Weekends & Holidays:While the department isclosed during these times, there will be rotating on-callresponsibilities during weekends and holidays. Job Summary: TheOutpatient Pharmacy Coordinator is the pharmacist-in-charge and managingpharmacist per the Wisconsin Pharmacy Examining Board who oversees dailyoperations and successful performance in the assigned location. Thepharmacist performs a combination of clinical and distributive pharmacyservices necessary for the provision of pharmaceutical care to eachpatient. To accomplish this objective, the pharmacist-in-charge shallprovide supervision and guidance to pharmacy team members in theassigned location. The pharmacist will participate in handling on-callresponsibilities, including the management of sick calls. The pharmacistshall collaborate with other members of the health care team toproactively identify and address opportunities to optimize drug conjunction with the Froedtert mission focusing on customer safety,service, confidentiality, education, and world class excellence; thepharmacist assures that patient care is maintained through compliancewith t he department's quality improvement initiatives and othermedication management regulatory standards established by The JointCommission, Center for Medicare/Medicaid Services, URAC, and others. Thepharmacist maintains accountability for other duties as assigned.EXPERIENCE DESCRIPTION: A minimum of one year of retail pharmacyexperience is required. A minimum of three years of other pharmacyexperience is preferred. EDUCATION DESCRIPTION: Bachelor's degree inPharmacy from an ACPE certified school of pharmacy or a Pharm. D. isrequired. Successful completion of ASHP Community Pharmacy Practice orSpecialty Residency is preferred. SPECIAL SKILLS DESCRIPTION:Demonstrates effective use of computers and applications (e.g. MS Word,Excel, Outlook) as appropriate to practice and communicate effectively.Exemplifies strong organizational, time management, leadership, conflictmanagement, and change management skills. LICENSURE DESCRIPTION: ActiveWisconsin Pharmacist License is required or eligible for and must obtainwithin 3 months. Active membership in the Pharmacy Society of Wisconsin(PSW) preferred. Active membership in the American Society of HealthSystem Pharmacists (ASHP), American Pharmacist Association (APhA) and/orother relevant national pharmacy organizations is preferred.
