As a Home Health Aide on our Hospice team, you will care for patients where they need you most, whether at home, a nursing facility, assisted living center or other location. In this role, you can help enhance their outlook on their experience, even when their futures seem uncertain. Every day, you can make a lasting difference in both the lives of your patients and their loved ones.A caregiver in this position works evenings from 10:00am to 7:30pm.A caregiver who excels in this role will:Perform basic patient care activities.Assist with procedures ordered by physician, with supervision from an RN, PT, OT or ST as appropriate.Assist patients with tending to personal care, activities of daily living and transfers/transport.Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff.Assist with keeping the patient's environment clean and orderly.Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.Escalate change in patient condition to patient’s physician as appropriate.Minimum qualifications for the ideal future caregiver include:High School Diploma or GEDState Tested Nurse Aide (STNA) Certification with the State of Ohio Nurse’s Aide RegistryOne year of clinical experience as an Aide in an acute, rehab or skilled nursing settingValid Driver’s License (VDL) in Ohio and proof of $100,000/$300,000 automobile insurance coverageBasic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross within orientation periodPreferred qualifications for the ideal future caregiver include:Hospice and Palliative Care Nurse Assistant (CHPNA) CertificationHome Care experienceOur caregivers continue to create the best outcomes for our patients across each of our facilities.
