Wound Care Nurse (RN) _Brooklyn, NY_ _Full-Time Skilled Nursing / Long-Term Care_ We are seeking a clinically strong Wound Care Nurse to lead skin integrity initiatives within a high-acuity Skilled Nursing facility in Brooklyn. This is a highly visible role that directly impacts resident outcomes, survey performance, and quality metrics. The ideal candidate is hands-on, detail-oriented, and confident in both advanced wound treatment and regulatory documentation. Position Overview The Wound Care Nurse is responsible for comprehensive wound assessment, treatment planning, documentation accuracy, and facility-wide education. You will collaborate closely with physicians, nursing leadership, rehab, dietary, and infection control teams to ensure best-in-class wound management and prevention. Key Responsibilities u2022 Conduct comprehensive skin assessments upon admission and routinely thereafter u2022 Stage and document pressure injuries accurately in accordance with NPIAP guidelines u2022 Develop and oversee individualized wound treatment plans u2022 Perform and supervise advanced wound care treatments and specialty dressings u2022 Lead weekly wound rounds with interdisciplinary teams u2022 Partner with MDS to ensure accurate coding and compliance u2022 Maintain survey readiness related to skin integrity standards u2022 Track wound metrics and participate in QAPI initiatives u2022 Educate nursing staff and CNAs on prevention protocols and documentation standards Qualifications u2022 Active, unencumbered RN license in New York u2022 2+ years of wound care experience in Skilled Nursing or Sub-Acute setting u2022 Strong knowledge of pressure injury staging and prevention u2022 Familiarity with MDS 3.0 and Quality Measures u2022 Experience with EHR systems (PointClickCare or SigmaCare preferred) u2022 Wound Care Certification (WCC, CWCN, or similar) preferred Why This Role Stands Out u2022 High-impact position tied directly to facility quality outcomes u2022 Clinically advanced resident population u2022 Collaborative interdisciplinary culture u2022 Opportunity to lead prevention initiatives and drive measurable improvement If you are a proactive, clinically confident nurse who takes ownership of outcomes and thrives in a fast-paced Skilled Nursing environment, we encourage you to apply.
