JOB REQUIREMENTS: Access Community Health Centers, Inc RegisteredNurse - Triage Regular Full-Time Registered Nurse - Triage PositionSummary: The Registered Nurse (RN) works collaboratively as an activemember of the interprofessional team to provide therapeutic patient andfamily centered care. The RN systematically and continuously collectsand assesses data related to patient health status to develop, execute,and evaluate the plan of care. The RN utilizes evidence to driveclinical decisions and practice related to individual and populationhealth. The RN establishes goals and strategies for meeting carecoordination needs and provide outreach services across settings withthe patient, family, and/or other care providers. The Registered Nurseeffectively delivers medical and disease education, while activelymanaging a patient's immediate and ongoing needs with attention toacute and preventative intervention The RN considers factors related tosafety, effectiveness and fiscal accountability in planning anddelivering care. The RN delegates and supervises care in accordance withthe WI Nurse Practice Act and ACHC Delegation Protocols. The RNparticipates in continuous quality improvement initiatives and assumesresponsibility for their professional development.
