Job Title: Ambulatory Nurse II Department: Rheumatology Outpatient Location: Perelman Center for Advanced Medicine 3400 Civic Center Blvd Entity: CPUP Hours: Mon-Fri 8am-5pm Office hours per department needs; Hybrid eligible after introductory period Summary: We are seeking a passionate, detail-oriented rheumatology RN to join our dynamic team. This is an exceptional opportunity to work alongside renowned rheumatologists, advanced practice providers, and interdisciplinary teams in a collaborative, intellectually stimulating environment. Responsibilities: The Rheumatology Clinical Nurse will be responsible for providing compassionate and high-quality care for patients who are followed in the Rheumatology Clinic. The ideal candidate will be comfortable working in a fast-paced high-volume outpatient clinical practice. The RN is responsible for triaging patients for their assigned providers with the following duties: u2022 Facilitating patient access to the clinic u2022 Triaging patient calls u2022 Reviewing and refilling prescriptions u2022 Responding to patient portal messages u2022 Providing patient education regarding disease and medication management u2022 Injection teaching/administration. This ambulatory nurse role will be primarily computer and phone based with some in person patient interaction. This role will be considered for a hybrid remote working model after successfully completing an introductory period. The RN is expected to handle the workload appropriately and in a timely manner, per clinical policy. The RN will work closely with the providers and administrative staff to provide comprehensive care for the patients. u2022 The Ambulatory Nurse II Colleague is an RN that practices as a full partner on the health care team and consistently demonstrates safe practice within the organizational professional practice model. u2022 The Ambulatory Nurse II Colleague possesses the knowledge, skills and the attitude to effectively meet standards for competent professional nursing practice as specified in the Ambulatory Nurse II position description addendum. u2022 The Ambulatory Nurse II Colleague exhibits UPHS organizational behavioral competencies which reflect its mission and are embedded in the professional practice competency domains. u2022 The Ambulatory Nurse II Colleague independently provides patient care through the application of the nursing process and accepts accountability for the nursing care of assigned patients. u2022 Assigned patients may range in age from infancy to the elderly. u2022 The Ambulatory Nurse II Colleague demonstrates within the competency domains of Continuous Quality Improvement, Evidence Based Practice and Research, Leadership, Person and Family Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork (Definitions listed below). u2022 The Ambulatory Nurse II Colleague works closely with the interprofessional health care team to facilitate the coordination of care across the continuum. u2022 The nurse focuses on expanding the knowledge and skills necessary to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age-appropriate considerations of assigned patients. u2022 Evidence Based Practice and Research: The nurse evaluates and integrates best current evidence with clinical expertise and patient and family preferences and values for the delivery of optimal health care and system effectiveness. u2022 Safety: The nurse minimizes risk of harm to patients, families, providers and self through system effectiveness and individual performance. u2022 Professionalism: The nurse demonstrates a commitment to the nursing profession through lifelong learning, adherence to ANAu2019s Code of Ethics for Nurses, participation in a professional organization and advancing community outreach. u2022 Person and Family Centered Care: The nurse recognizes the patient (or the patientu2019s designee) as the source of control and a full partner in providing compassionate and coordinated care based on respect for the patientu2019s preferences, values, and needs. u2022 Leadership: The nurse effectively collaborates and applies innovative, systems thinking to engage in systematic, evidence-based problem solving and decision making to promote effective changes within a complex care delivery system supporting the vision of Penn Medicine. u2022 Informatics/Technology: The nurse utilizes appropriate information and technology to communicate, manage knowledge, mitigate error, and support decision making across the continuum. u2022 Teamwork: The nurse effectively engages in the process of cooperation, coordination, and collaboration in an effort to provide for safe, quality outcomes for patients within inter and intra-professional teams, including virtual teams. u2022 Continuous Quality Improvement (CQI): The nurse utilizes data and QI methods to identify potential and actual problems and opportunities to provide care that is safe, timely, efficient, effective, and equitable. u2022 Performs duties in accordance with Penn Medicine and entity values, policies, and procedures. u2022 Other duties as assigned to support the unit, department, entity, and health system organization. Credentials: + BLS/ACLS/PALS; per specialty (Required) + Registered Nurse - PA (Required) + Certification in specialty (Preferred) Education or Equivalent Experience: + Diploma or ASN from an accredited school of nursing and more than 15 months of relevant professional nursing experience. (Required) + BSN from an accredited school of nursing and more than 15 months relevant professional nursing experience.
