A Brief Overview The LIMHP Bridge Care Therapist provides short-term, solution-focused therapy to pediatric patients and families awaiting ongoing outpatient or psychiatric care. This position bridges the gap between referral and long-term treatment, ensuring timely access, stabilization, and continuity of behavioral health services. The therapist collaborates closely with Behavioral Health Intake, Outpatient Therapy, and Psychiatry teams to promote patient safety, support transitions, and reduce wait times. Essential Functions + Provide brief, evidence-based therapy (typically 1u20138 sessions) focused on stabilization, coping, and connection to ongoing care. + Conduct assessments, develop treatment plans, and ensure timely documentation in alignment with clinical and regulatory standards. + Ongoing monitoring and updating patient/family safety plans. + Coordinate with Intake, Psychiatry, and community partners to ensure warm handoffs and continuity of care. + Participate in case consultations, quality improvement, and outcome tracking to strengthen Bridge Care program effectiveness. + Support crisis intervention, safety planning, and provider consultation as needed.
