Essential functions (job duties): Job Purpose: The Peer Navigator serves as a member of a multidisciplinary clinical care team and provides peer-based supportive services to people living with Human Immunodeficiency Virus (HIV) or those affected by HIV. The Peer Navigator utilizes lived experience, trust-building, and community engagement to support patient retention in care, reduce barriers to care, and promote overall health and well-being. This position will report to the Manager, Psychosocial Team. Essential Functions: Provide peer-based support, engagement, and navigation services to patients living with HIV, including newly diagnosed individuals, including those returning or experiencing barriers to care. Assist patients with Ryan White Enrollment and Eligibility processes, Health Insurance Enrollment, and medical transportation coordination, in collaboration with Patient Advocates and Patient Financial Counselor. Conduct routine outreach, follow-up, and retention activities through phone calls, in-person contact, and community-based engagement to support appointment adherence and continuity of care. Assist Early Intervention Services (EIS) in collaboration with the EIS team and support patients during and after EIS engagement activities as appropriate. Facilitate and participate with peer support groups, outreach activities, community engagement initiatives, including participation with community advisory boards, and special events. Document services and activities in the electronic medical record (EMR), other required reporting systems and collect program data. Perform other related duties as assigned. Unique duties: Qualifications: Education: High school completion (or GED) Experience: Experienced (minimum 2 years of job-related experience) Experience working in community outreach, peer support, patient navigation, public health, or social service settings is preferred. Familiarity with medical terminology preferred. Experience related to HIV patient care and/or equivalent experience. Knowledge, Skills, And Abilities: Ability to provide peer-based support using empathy, lived experience, and effective communication. Ability to build trust and rapport with diverse patient populations. Understanding of barriers faced by individuals living with HIV. Ability to work collaboratively within a multidisciplinary clinical care team, including EIS staff, Patient Advocates, Patient Financial Counselor. Social Workers and Physicians. Strong interpersonal and customer service skills. Ability to maintain appropriate professional boundaries. Ability to document services accurately in the electronic medical record and other required systems. Organizational skills and ability to prioritize tasks. Ability to engage in community outreach and group facilitation. Basic knowledge of Microsoft Office applications.
