Job Summary The Family Medicine/Urgent Care AdvancedPractice Provider (APP) at Prevea is a Nurse Practitioner (NP) orPhysician Assistant (PA) prepared to deliver comprehensive health careservices to patients, families, and groups. This role emphasizes healthpromotion, disease prevention, and the management of acute and chronicconditions across all ages. As a Float APP, this position providesflexible support across Primary Care and Urgent Care locations,delivering both primary and prompt care for illnesses and injuries thatrequire timely attention but do not necessitate emergency departmentvisits. The APP functions with a high degree of autonomy whilecollaborating and coordinating with physicians and other healthprofessionals to ensure high-quality, patient-centered care.Responsibilities also include participation in clinical practice,education, consultation, research, and program development initiativeswithin the Prevea Health system. What you will do Provides assessment,evaluation, and management to patients with minor, acute, chronic, &episodic illness & injuries. Including performance of follow up exams ifrequired. Collaborates with patients to review their health backgroundand determine physiological, emotional, and environmental needs, usingthis information to advise patients and their families on future healthplans and illness prevention. Provides case management services toassist clients and families with the arrangement for, or refer patientsto needed services. Provides services, which may include the ordering ofappropriate diagnostic and laboratory tests, the prescription ofpharmacological agents, and other non-pharmacological treatmentmodalities. Also at times conducting Maintains and reviews patientrecords, charts, and other pertinent information, documenting alldetails in the electronic medical record. Conducts telephone calls andelectronic communications back to patients requiring follow-up care orprovide test results.
