Summary There is a full time opening for a Registered Nurse in the Primary Care Clinic in Green Bay. The function of the Patient Aligned Care team (PACT) RN Care Manager is to deliver evidence-based practice and patient driven holistic care. Responsibilities The RN Care Manager is a member of PACT and will function as an integral member of the interdisciplinary team in managing chronic diseases. The RN must demonstrate outstanding communication skills with the ability to triage patient concerns and complaints in person or via telephone. The RN in the role of Care Manager applies the principles of advanced clinic access and PACT principles within their daily work. The RN demonstrates advanced clinical expertise in the nursing process (assessment, diagnosis, planning, implementing, and evaluation) when caring for the veteran population. The RN possesses broad-based clinical knowledge and skills to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary practice setting. The RN effectively implements activities towards health promotion and disease prevention along with the management of acute and chronic illness.
