FTE: 1.000000Standard Hours: 40.00 Shift: 4 Shift Details: All shifts, includingoccasional third shift Holidays: Yes Weekends: Yes Job Summary: Usingindependent decision making and critical thinking skills, the Pharmacistpromotes medication use that is safe, effective and considers the costsand benefits of therapy to maximize value. The Pharmacist interacts withother members of the healthcare team and with patients to proactivelyaddress medication therapy issues. The Pharmacist ensures patient careis maintained through compliance with the department's qualityimprovement plan, and the Joint Commission/Center for Medicare/Medicaidstandards for pharmaceutical services. EXPERIENCE DESCRIPTION: A minimumof 1 year hospital or 3 years of other pharmacy experience is preferred.EDUCATION DESCRIPTION: Bachelor's degree in Pharmacy or Pharm.D. froman ACPE certified school of pharmacy is required. Successful completionof an ASHP accredited PGY1 or specialty residency is preferred. SPECIALSKILLS DESCRIPTION: Demonstrates effective use of computers andapplications (e.g. MS Word, Excel, Outlook) as appropriate to practiceand communicate effectively. Active membership in the Pharmacy Societyof Wisconsin (PSW) preferred. Active membership in the American Societyof Health System Pharmacists (ASHP) and/or other national pharmacyorganizations as approved by the Pharmacy Administration preferred.LICENSURE DESCRIPTION: Active Wisconsin Pharmacist License is requiredor eligible for and must obtain within 6 months.
