Details Department:Care Coordination Command CtrSchedule:FT Monday-Friday Shifts- 7 AM- 7 PMHospital:St. VincentLocation:Indianapolis, INThe RN Discharge Expeditor role focuses on the Clinical Progression of Care, Identifying barriers, prioritizing needs and working with the interdisciplinary teams to facilitate patient throughput. Monitor, record, and communicate patient condition as appropriate.Serve as a primary coordinator of all disciplines for well-coordinated patient care.Note and carry out physician and nursing orders.Assess and coordinate patient's discharge planning needs with members of the healthcare team.Requirements Licensure / Certification / Registration:BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.Education:Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.Additional Preferences Recent bedside experience is requiredMed Surg, Progressive Care, Resource, Ortho, PCCU or MPCU experience preferred
