We’re hiring Registered Nurses (RNs) and Nurse Leaders across our Continuing Care division, which includes Residential Services and Community Services (Home Care & Hospice). This evergreen posting supports ongoing recruitment across multiple settings as needs arise.Residential Services (Skilled Nursing / Rehab)Provide skilled nursing care, clinical oversight, and care coordination in a long‑term care or rehab mon RN & RN Leadership focus areas:Resident assessments and care planningMedication administration, treatments, IV therapy (as applicable)Supervision and clinical guidance of LPNs/CNAsCollaboration with providers and interdisciplinary teamsFamily communication and educationCommunity Services (Home Care & Hospice)Deliver professional nursing care in patients’ homes and community settings—supporting recovery, chronic disease management, and end‑of‑life comfort.Home Care RN focus areas:OASIS/assessment (as applicable), care planning, and visit-based careCoordination with physicians and care teamsPatient/caregiver education and disease managementHospice RN focus areas:Symptom management and comfort-focused careEmotional support and education for patients and familiesCoordination with interdisciplinary hospice teamLocations HiringMultiple Continuing Care locations across the region (varies by division and need)What We OfferCompetitive pay and shift differentials (where applicable)Full‑time, part‑time, per diem optionsComprehensive benefits (medical, dental, vision, retirement)Tuition reimbursement and career development and progressionSupportive leadership and a mission‑driven cultureOpportunities to grow into charge, educator, case management, or leadership pathways (based on division)What You Will Do (Core RN Expectations)Across Continuing Care settings, you will:Perform comprehensive assessments and document clinical findingsDevelop, implement, and evaluate individualized plans of careAdminister medications and treatments per orders and scopeIdentify changes in condition and act promptly to ensure safetyCollaborate with interdisciplinary teams to optimize outcomesProvide education and emotional support to residents/patients and familiesEnsure compliance with policies, procedures, and regulatory standardsKey Responsibilities (By Setting)Residential: clinical oversight, delegation/supervision, care conferences, quality/safetyHome Care: visit-based care, coordination, patient/caregiver training, documentationHospice: symptom control, psychosocial support, comfort measures, interdisciplinary coordinationSenior Living: wellness support, care coordination, incident response, resident educationQualificationsCurrent New York State RN license in good standingStrong assessment, critical thinking, and communication skillsCompassionate, patient-centered approach across the continuum of careAbility to work independently (especially for home care/hospice roles)Preferred (Not Required)Skilled nursing or rehab experience (Residential Services)Home care and/or hospice experience (Community Services)Case management, wound care, or geriatric experience (helpful across settings)Valid driver’s license and reliable transportation (for home-based roles)Ready to Apply?Apply today to join our Continuing Care teams. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
