Assists with the coordination of all aspects of patient care with home health clinicians, physicians, and any other patient care/service providers. Provides clinical support to the workflows in Community Home Care to ensure quality patient care.EXPERIENCE AND EDUCATION:Graduation from an accredited School of Nursing or successful completion of sufficient units in Nursing to qualify for state licensure. One year of clinical experience as a Registered Nurse.LICENSES/CERTIFICATIONSCurrent California Registered Nurse license. Current BLS certification.
