FTE: 1.000000 Standard Hours: 40.00Shift: Shift 1 Shift Details: Holidays: Weekends: Job Summary: TheVAD/Heart Transplant Coordinator will be responsible for thecoordination of care for all patients requiring implantation of acardiac assist device. These include patients accepted for, and awaitingheart transplant, or those with Advanced Heart Failure in which thedevice is implanted as a permanent treatment option. The VAD/HeartTransplant Coordinator will also be responsible for the coordination ofcare for all patients who are pre- or post- heart transplant. TheVAD/Heart Transplant Coordinator will work as a member of the AdvancedHeart Failure Team, and will be the primary liaison with the transplantcardiologists and cardiac transplant surgeons, other physicians, andhealth care staff throughout the hospital. A major component of theposition is patient and family education, daily rounds, dischargeplanning, and on-call coverage. The VAD/Heart Transplant Coordinator isalso responsible for outreach activity to promote the programs, frequentcommunication with ref erring physicians, and professional education.The VAD/Heart Transplant Coordinator functions within the parametersoutlined in the LVAD practice guidelines. The Job Standards portion ofthis job description has incorporated the 17 standards adapted from theANA Nursing: Scope & Standards of Practice, 2nd Edition (2010). Otherduties as assigned. EXPERIENCE DESCRIPTION: Minimum of 3 years of acutecare or experience is required. Experience relevant to acute care,cardiology, heart failure, VAD care is preferred. EDUCATION DESCRIPTION:Associate's degree in Nursing with a minimum of 10 years of relevantexperience is required or Bachelor's degree in Nursing with a minimumof 3 years of relevant experience is required. Bachelor's degree inNursing is preferred. SPECIAL SKILLS DESCRIPTION: Analytical skillsnecessary in order to prepare and implement effective nursing care anddevelop solutions to problems concerning patient's mental or physicalwell-being. Ability to concentrate and pay close attention to detail forup to eighty percent of work time while: planning and performingprofessional nursing care, resolving patient care problems and dealingwith patients, their families, and other professionals involved in thepatients care, often during stressful situations. LICENSURE DESCRIPTION:Requires current state of Wisconsin Registered Nurse License or aMulti-state Nursing License from a participating state in the NLC (NurseLicensure Compact). Certified Clinical Transplant Coordinator (CCTC)certification, Certified Clinical Transplant Nurse (CCTN), orAcute/Critical Care Nurse (CCRN) is preferred.
