As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants ofRochester, NY (and in Canandaigua, N.Y. Jordan Health is now seeking aCommunity Engagement Program Coordinatorwho, under the guidance of the Chief of Communication and Engagement Officer, is primarily responsible for advancing Jordan Healthu2019s presence in focused patient communities through outreach activities and community-based organization partnership. Requirements TheCommunity Engagement Program Coordinatorwill ensure that: u00b7 Awareness of Jordan Health priorities is broadened and the visibility of Jordan Health programs within targeted audiences is increased. u00b7 Diversity of the services most used by (the focused community group) coming to Jordan Health for health care are increased. u00b7 Community engagement programs and activities are cost effective and demonstrate full support of Jordan Healthu2019s strategic community engagement goals. Education And Experience Required: u00b7 High School diploma and post-secondary education; Associates preferred. u00b7 3-5 years of community organizing and demonstrated community engagement/outreach experience or equivalent combination of education and experience. u00b7 Demonstrated ability to relate and communicate with diverse populations and groups. u00b7 Effective computer/internet skills u00b7 Knowledge and understanding of the Microsoft Office suite of software, specifically Excel, Word, and PowerPoint. Certifications And Licenses u00b7 BLS through the American Heart Association u00b7 NYS Infection Control Certificate u00b7 Valid Driver license and reliable, available transportation Special Skills, Knowledge Required: u00b7 Demonstrate experience, ideally in the community health arena, in: 1. Community-based organization partnership development; 2. Community meeting facilitation; 3. Event planning and promotion; 4.
