Salary Range $25.00 - $27.00 Hourly Level Experienced Position Type Full Time Job Shift Day Travel Percentage None Description Position Statement: Under supervision of and consultation with the RN nurse manager, The LPN is responsible for ensuring efficient patient care within the medical department. The LPN provides guidance for the nurses in accordance with their level of training and works in cooperation with medical providers to ensure the quality of patient care. Responsibilities: + Ensures nursing staff are competent in their job responsibility through supervision and observation of required tasks. Will oversee the care provided by both the RN and LPN during shifts(s) when needed. + Complete training of any new employees by successfully orientating to the job responsibilities/tasks/and requirements within the Medical Department + Completes medication error/incident forms during shift and reports all medication errors to the appropriate management personnel. + Respond to Residential needs and issues involving medical department + Communicates problems involving personnel, medical staff, patients, to the appropriate management personnel + Ensures accurate and complete medical records are documented and scanned, including any EHR, Connections and Welligent. + Provide on-call coverage for triage and nursing staff needs 2-3x a month.u00a0 Provide weekend on-campus coverage only as needed during on-call coverage. + Implements emergency procedures as indicated + Complies with agency policies and procedures + Perform duties and responsibilities as the Vaccine Coordinator by completing requirements such as ordering and purchasing vaccines, returning expired vaccines, documenting temperature logs in NYSIIS, upload data loggers to NYSIIS, understanding emergency plan for transport of vaccines if needed, and admitting and discharging youth on NYSIIS for Elmcrest Childrenu2019s Center. + Perform weekly audit on medical supplies and completes an Elmcrest requisition form when needed. Order form to be submitted to Nurse Manager on a weekly/bi-weekly basis and/or as needed for approval. + Provides patient/family education + Has working knowledge and demonstrates an understanding of the NYS Justice Center + Provide screenings to youth with illness, injury, or health related concerns. Reports any significant findings to the appropriate provider. + Transcribes all information accurately and efficiently and carries out orders for treatment as medically prescribed. + Administer medication(s) safely, accurately, and per best practice, NYS and agency guidelines. Is attentive in monitoring for any adverse effects. + Provide monitoring, and care to the youth in the event of a physical restraint per agency requirements for injuries, attend residential restraint committee meeting. + Trains and mentorsu2019 new staff on evenings when needed. + Ensures smooth shift handovers by filling out report sheet accurately + Administer first aid during emergency situations and coordinate patient care until EMS arrives or the youth is transported to the emergency department + Coordinate with members of the treatment team providers such as the MD, NP, Psychiatrists, Clinicians, as well as others as needed to ensure continuity and quality of care. + Assure that the physical health needs of each youth are met in a timely and efficient manner + Vaccine administration + Phlebotomy + Clerical work + Will schedule providers schedule and make sure all charts are ready for doctor day. + Transporting youth to medical/ dental appointments when transport is unavailable. + Other duties as deemed necessary by Nurse Manager + In essence, ensure the unit runs smoothly by balancing oversight with hands on leadership. Qualifications + Competencies : + Demonstrates Competency and Professional Responsibility in the Charge Nurse Role + Be knowledgeable in the medical regulations for OCFSu00a0and OMH + Communicates problems involving personnel, medical staff, patients, to the appropriate management personnel + Must have proven ability to maintain professional conduct and confidentiality in the care of patients + Required Education and Experience : + Licensed Practical Nurse (New York State license/registration) + Pediatric Experience for 2 years preferred + Experience with psychiatric care within the last two years + Additional Eligibility Qualifications : + Experience with EHRs and other database, ordering and tracking systems. + CPR Certified Essential Job Qualifications: + Ability to lift 25 + pounds without assistance. + Ability to sit, squat, crouch and stand for elongated periods of time + Ability to lift arms over head in upward reaching motion. + Ability to walk up and downstairs up to several times per day. + Varying levels of travel as determined by departmental needs may be required with this position.
