Unlock your potential at Heart Hospital of Austin!Job Summary and QualificationsPerforms direct patient care within the scope of the nurse practice act, as it applies to the outcomes and process in the Valve Clinic. Plans, coordinates and implements the care of the valve patient in the outpatient setting. Scope of the role includes acting as a liaison with physicians, patient/familiesand ancillary departments. Assessment of financial,medica land social resources of current and potential valve program patients in conjunction with the research team. Coordination of information flow among the medical, nursing, and financial and research staff asappropriate. Development and implement a teaching plan with the patient and family. To include lab valves, medicationoverviewand diagnosis related information.Assistthe research team in the coordination of patient charts, diagnosticstudiesand other applicable information per protocol.What you will do:Work with supervisor to plan, coordinate, and implement care of the valve patients in the outpatient clinic.Review clinical progress of patients andcoordinatescommunicationstoreferring,valve and primary care physicians along with the research team.Plan,organizeand provide direct care to the valve patient.Facilitate the development of apatient specificteaching plan.Review patient’s current health information, relayinformationand makereferrals asnecessary to the physician and research partners.Serve as a resource for the hospital staffregardingthe care of the valve patient.Educate patient and familiesregardingplanned procedures,medicationsand treatments.Make referral asappropriate asnoted by the physicianCoordinate information among:Referring, primary care and valve clinic physiciansResearch teamAncillary departmentsAnswer patient/family phone calls, field questions appropriatelyParticipate in creating an environment that implements the mission, vision, and values of the organizationEncourages collegiality when integrating valve services with other departments.Responsible for full nursing assessment of clinic patients, including documentation of vital signs,signand symptoms of disease process,NYHAand angina class.Responsible for reviewing all diagnostic imaging studies, labvaluesandnotifyMD asappropriate forabnormal findings.Knowledge and application of pertinent computer programs and imaging softwareWhat qualifications you will need:Basic Cardiac Life Support must be obtained within 30 days of employment start date(RN) Registered NurseAssociate Degree, or Bachelors Degree, or Registered Nurse DiplomaAt least 3 years of cardiology or critical care experience.BenefitsHeart Hospital of Austin, offers a total rewards package that supports the health, life, career and retirement of our colleagues.
