Summary The VA Finger Lakes Healthcare System is recruiting for a Full time Advanced Practice Nurse for our Community Living Center (CLC) in Bath, New York. The Finger Lakes Healthcare System offers a wide range of health, support, and facility services for Veterans in upstate New York and northern Pennsylvania. Eligible applications received will be referred at regular intervals or as additional vacancies occur on an as-needed basis until positions are filled. VA offers a comprehensive total rewards package: VA Nurse Total Rewards The Bath VAMC Community Living Center (CLC) is comprised of multiple units with a mix of short stay (less than 90 days) and long term (over 90 days) Veteran residents. The CLC includes the following patient care mix: Hospice Care, Dementia Care, Mixed Medical/Long-Term Care/Skilled Nursing, Short Stay Rehabilitation Care, and Respite Care. Major duties and responsibilities of the position include but are not limited to: Independently manages a panel of patients/residents with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with the interdisciplinary team. Participates in the review of admissions to clinical program. Participates in the discharge planning process. Completes admission H&Ps, tends to the day-to-day clinical management needs of the Veteran, reassessments, discharge summaries (CLC), etc. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Provides occasional cross-coverage in program area as needed.
