Director of Nursing (DON) Full-Time Skilled Nursing / High-Acuity LTC Utica, NY A high-acuity Skilled Nursing Facility in Utica is seeking an experienced, survey-ready Director of Nursing to lead all clinical operations. This is a hands-on leadership role for a strong RN who understands the regulatory intensity of New York State and takes full ownership of building performance. This facility manages complex residents including vents, dialysis, sub-acute rehab, advanced wound care, and medically fragile populationsu2014making it ideal for a DON comfortable in fast-paced, high-expectation environments. Key Responsibilities + Oversee all nursing services and maintain NYSDOH/CMS compliance + Lead survey readiness and Plans of Correction + Drive staffing stability and unit-level leadership development + Monitor labor management and high-acuity staffing models + Lead QAPI initiatives targeting readmissions, infection control, falls, and star ratings Requirements + Active NY RN license + 5+ years LTC experience + 2+ years as DON or ADON + Strong NY DOH survey and MDS knowledge + Experience in high-acuity Skilled Nursing settings Ideal for a visible, accountable DON who thrives under regulatory pressure and is ready to elevate clinical standards in a complex LTC setting.
