Summary This is an intermittent/PRN position with Community Based Clinics. This RN will demonstrate proficiency in triage, assessment, interdisciplinary team communication, and care coordination to improve patient outcomes. Responsibilities The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing competent, evidence-based gender-specific care to primary care patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. The nurse performs triage and assessment of patients on the team panel or unit making sure to provide prompt and appropriate treatment for Veterans health concerns to ensure care is timely. The nurse demonstrates high quality customer service skills. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. This nurse provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. This includes managing and monitoring high risk patients on the panel for improvement of patient outcomes including prevention of hospitalization and improvement in ambulatory care sensitive conditions. The nurse collaborates and coordinates care with interdisciplinary team members from other services within the department as well as services in the community. The nurse supports virtual care clinic modalities. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhance outcomes utilization. This nurse integrates outpatient nursing knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex PACT situations. Validates understanding of the rationale for care processes and decisions. Questions clinical practices for the purpose of providing evidence-based care. The nurse will demonstrate the ability to cope with and manage competing priorities. The nurse demonstrates competence in primary care practice and decision-making, deliberate planning, and critical thinking skills. Responsible for participating within unit based and/or organizational level Shared Governance councils, utilizing clinical expertise, evidence-based practice, and knowledge to assist with finding solutions to problems or issues to improve the quality care provided to our veterans. Work Schedule: Intermittent/PRN, Monday-Friday (Day Tour) Telework: Not eligible for telework. Virtual: This is not a virtual position. Requirements Conditions of Employment U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
