Summary REACH Vet/ Suicide Prevention Team Case Manager/ CBH RN is responsible for providing competent, evidence-based care to assigned patients. The RN will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation. Responsibilities Major Duties: CBH RN CBH RN independently provides patient-centered and recovery-oriented care and maintains self-awareness while caring for patients living with MH conditions. They facilitate warm hand-offs, telemental health care, and nurse-led psychoeducational groups; use therapeutic communication techniques, practice de-escalation techniques, utilize recovery-oriented language, and provide Veterans, their family members, and/or significant others with education regarding their illness, medications, and treatment plan. The MH RN actively participates in care coordination, liaising with Inpatient MH Units to facilitate continuity of care, MH interdisciplinary treatment team huddles and meetings and responds to psychiatric and medical emergencies as directed. The MH RN reports and addresses safety concerns, document nursing triage assessments, risk screens, clinical reminders, and patient plan of care, according to facility policy. The MH RN maintains professional boundaries to protect patient vulnerabilities and act in the best interest of the patient. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Supports colleagues through knowledge sharing to provide safe, quality nursing care. Questions clinical practices for the purpose of providing evidence-based care. Suicide Prevention/REACH VET Coordinator: Suicide Prevention/REACH VET Coordinator in Behavioral Health (BH) demonstrates leadership in delivering efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication, and coordination of resources. The incumbent works in partnership with The Suicide Prevention Team and collaborates with all staff. The work of this position is highly complex, involving a combination of mental health administrative skills and intensive clinical services both of which require the exercise of mature professional judgment and the flexible use of a wide range of professional abilities. The incumbent is able to provide clinical services in serious and complicated cases and carries full professional responsibility for those cases presenting with a wide range of psychosocial and environmental problems. The RVC-SPC is capable of practicing at the advanced level and has specialized knowledge of practice related to suicidal risk management and treatment. The RVC-SPC should have at least 2 years of behavioral health experience, preferably in suicide prevention. Serves as subject matter expert on Suicide Prevention, mental health intervention and crisis management for VA Butler leadership, staff, and volunteers. Provides and coordinates education and consultation to all VA Butler staff (VA and contract) throughout the Medical Center and at all residential and outpatient locations. Provides facility suicide prevention oversight, education, and community outreach on topics of veteran self-directed violence, suicide prevention intervention and the public health approach to suicide prevention. Responsible for tracking and reporting of all self-directed violence events, continued contact and follow-up with veterans identified as being at high risk for self-directed violence. Facilitate professional development of clinical staff, supported staff empowerment and development of high functioning suicide prevention teamwork within the facility. This clinical position includes a level of physicality normally associated with direct patient care duties performed by nurses, such as potentially long periods of continued walking, standing, sitting, moderate lifting, repeated bending, or kneeling. Examples may include but are not limited to: Significant amounts of time standing assisting assigned patients. Utilization of safe patient handling and mobility protocols.
