We are connected by our mission - partnering and healing for a healthy future - and dedicated to the health and well-being of the communities we serve.FTE: 1.0, Shift: 7:00pm - 7:30am, Schedule: 3-12's per week, variable days, rotating nights & weekends.HH/Hospice RN - Night Shift - After Hour Visiting NurseTriage after hour phone calls from patients/referral sources/providers in both Home Health and Hospice.Determine in the triaging process if the patient needs a home visit and complete necessary nursing visits.Able to work independently in home settings; nursing home settings; assisted living settings coordinating care with caregivers and family members.Able to use teach back skills and other tools to engage the patient and family in goal setting and self-management.Able to support difficult decisions for patients/families such as end of life (Hospice) services.Document services performed per agency policy in the electronic medical municate with the day shift triage and the patient's case manager about the phone call/visit and follow-up plete phone calls for tuck-in or follow-up for newly admitted patients.Education, Experience and LicensureLicensed in the State of Washington as a Registered NurseCurrent CPR CertificationHome Health or Hospice experience preferredCurrent WA state driver's license and proof of insuranceAdditional Requirements:Car for travel to and from home visits during shift.Position SummaryWe're seeking a staff RN to join our incredible hospital-based team.
