The Pharmacy Population Health Coordinator supports clinical pharmacy programs within the Beth Israel Lahey Health Performance Network (BILHPN) u2013 Clinical Excellence Division. The role focuses on improving medication adherence, closing care gaps, and enabling performance in our valueu2011based payer contracts. Working under the direction of Clinical Pharmacy Specialists and pharmacy leadership, the coordinator carries out program operations, supports populationu2011healthu2011related pharmacy activities, and collaborates with interdisciplinary teams across the network. Job Description: Essential Duties & Responsibilities including but not limited to: u2022 Review medication adherence, quality, and performance reports to identify patients needing intervention. u2022 Conduct outreach to patients, providers, pharmacies, and care teams to address adherence gaps, medication access issues, and pharmacy benefit questions. u2022 Assist with addressing barriers to medication access including coordination of refills and prescription assistance programs. u2022 Route prioritized intervention opportunities to clinical pharmacists based on workflow and risk criteria. u2022 Support pharmacy-led population health and quality improvement initiatives. u2022 Perform administrative and operational tasks allowing clinical pharmacists to practice at top-of-license. u2022 Document all activities in the electronic medical record per established workflow. u2022 Participate in program data tracking, reporting, workflow optimization, and resource development. u2022 Participate in the education of PGY1 and PGY2 pharmacy residents and pharmacy students where applicable. Organizational Requirements: u2022 Maintain adherence to confidentiality policy u2022 Comply with BILH/BILHPN policies u2022 Maintain required certifications and competencies u2022 Maintain courteous and effective interactions with colleagues and patients. u2022 Participate in all department meetings Minimum Qualifications: Education: High school graduate with college degree preferred. Licensure, Certification & Registration: Certified Pharmacy Technician in Massachusetts; willing to obtain additional state licensure as needed. Experience: 2 to 5 years of experience in retail pharmacy, managed care, or population health environment. Skills, Knowledge & Abilities: u2022 Strong communication, time management, and organizational skills. u2022 Ability to work independently within scope of work. u2022 Experience with medication adherence, valueu2011based care contracts, and motivational interviewing skills preferred. u2022 Ability to communicate effectively with healthcare professionals and patients in person and telephonically. u2022 Ability to research and analyze medical literature, drug information, evidence-based guidelines and pharmacy, and cost utilization data. u2022 Knowledge of HEDIS, Stars Quality metrics preferred. u2022 Knowledge of Massachusetts / national Prescription Assistance Programs. u2022 MS Office applications including Word, PowerPoint, Excel and Outlook required; Experience with databases preferred. u2022 Experience with Epic Electronic Medical Record preferred. Pay Range: $27.01 - $36.35 The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
