Responsible for providing prescription delivery service to patients in a variety of care settings such as: the hospital, Emergency Department, or infusion room setting. Provides multi-disciplinary support to pharmacists, nurses, case managers, and other staff to promote an effective transition of care. Identifies patients eligible for delivery service, reviews drug benefit information for patients, third parties and HMOs to promote a smooth medication procurement process and to ensure reimbursement requirements are met. On our team, you will provide stellar care at one of the top healthcare organizations in the nation.The Pharmacy Technician - Hospital Care at Home supports a growing, innovative care model at Fairview Hospital that allows selected inpatient-level patients to receive care in their home. Working within a 24/7 pharmacy department supported by a large team of pharmacy technicians, this role prepares medications for Hospital Care at Home patients, reviews patient charts and collaborates closely with pharmacists and a centralized care hub based in Florida. Although patients are considered inpatients, they are typically low acuity and supported by a full interdisciplinary care team. This position offers pharmacy technicians an opportunity to work independently, expand their skill set and gain valuable experience while supporting a newer practice model in a dedicated medication preparation area within the pharmacy.A caregiver in this position will work a 7-on and 7-off schedule. Shifts are 10:00am to 8:30pm on 5 - 10-hour shifts and 2 - 11-hour shifts. If a holiday falls on their 7-on 7-off schedule, they will work that holiday. No on-call expectations.A caregiver who excels in this role will:Communicate directly with patients and caregivers while performing technical functions within the assigned work area to ensuretimelyandaccurateprocurement of prescriptions and documentation within the electronic medical record whenrequired.Identifypotential medication-related access problems andworkwith patients and the multi-disciplinary team to solve these problems in an efficient and patient-centered manner.Coordinate education, bedside medication delivery, and medication payment.Assistwith the development and implementation ofpolicies andprocedures to ensure a seamless medication-related procurement process for patients.Work with the pharmacy leadership teams toidentifyareas of opportunity for improved patient experience.Assistmanager in creating,implementingandmonitoringquality control programs in assignedarea.Conduct third partypriorauthorization requests when needed.Monitorprogram performance, including patient volumes served and patient experience indicators, serving as a valued resource toidentifydischarge prescription coverage issues andassistin their plete medication histories or other transitional care activities as assigned per location.Minimum qualifications for the ideal future caregiver include:High School Diploma/GEDSix months of related experience OR completion of aPharmacyTechnician ProgramUpon hire, registration as a Certified Pharmacy Technician, Registered Pharmacy Technician or Pharmacy Technician Trainee with the Ohio Board of Pharmacy is required. Candidate must be eligible for Registration as a Certified Pharmacy Technician with the Ohio Board of Pharmacy. Registration as a Certified Pharmacy Technician is required within 6 monthsCertified Pharmacy Technician (CPhT) through the Pharmacy Technician Certification Board (PTCB) is preferred at time of hire. CPhT through the Pharmacy Technician Certification Board (PTCB) is required within 18 monthsSuccessful completion of a Pharmacy Technician Program may substitute for the required work experiencePreferred qualifications for the ideal future caregiver include:Completion of a formal pharmacy technician training program preferred.Experience dealing with third party insurance and prior authorization processPhysical Requirements:Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.Manual dexterity sufficient to perform computer and dispensing functions of the job.Ability to lift up to 20 pounds.Position often includes reaching, stretching, stooping, climbing steps and sitting.Personal Protective Equipment:Follows standard precautions using personal protective equipment as required.Pay RangeMinimum hourly: $19.25Maximum hourly: $28.00The pay range displayed on this job posting reflects the anticipated range for new hires.
