Job Title: Registered Nurse (RN) Case ManagerJob Description We are seeking a compassionate and highly skilled RN Case Manager to join our dynamic team of care coordinators within an acute care setting. This role is pivotal in ensuring seamless transitions of care and optimizing patient outcomes across a variety of specialized units, including the Emergency Department (ED), Intensive Care Unit (ICU), Inpatient Treatment Areas (ITA), Pediatrics, and other Specialty Units. As part of a collaborative unit of Case Managers, the RN will focus on care coordination and discharge planning, guiding patients and families through the continuum of care and facilitating movement to the next appropriate level of care. Responsibilities + Collaborate with interdisciplinary teams to assess, plan, implement, and evaluate patient care needs. + Coordinate discharge planning and transitions of care, ensuring safe and timely movement to post-acute settings. + Serve as a liaison between patients, families, providers, and external agencies to support continuity of care. + Identify barriers to discharge and work proactively to resolve them. + Maintain accurate documentation in accordance with regulatory and organizational standards. + Participate in daily multidisciplinary rounds and unit huddles to align care plans. + Advocate for patient needs while balancing clinical, financial, and operational considerations. Essential Skills + Current RN license in WA State or Compact License (CCM or ACM) + Basic Life Support (BLS) certification + Minimum of 1 year of acute care nursing experience + Strong knowledge of discharge planning, utilization review, and care coordination principles + Excellent communication, critical thinking, and organizational skills + Familiarity with EMR systems and documentation standards Additional Skills & Qualifications + Experience in case management within an acute hospital and/or long-term acute care/rehab/skilled nursing setting + Proficient with EPIC, utilization review, care coordination, and discharge planning + Preferred experience in ED, Labor & Delivery, NICU, and pediatrics Work Environment This role is based in a Level II Trauma Center with 571 total staffed beds across both campuses. The position follows a 5x8 schedule from 7:30 AM to 4:00 PM. Attire is business casual, and parking costs are covered by the facility. Job Type & Location This is a Contract position based out of Seattle, WA. Pay and Benefits The pay range for this position is $45.00 - $55.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
