Summary The Cincinnati VA Medical Center is seeking 2 RN Hematology Oncology Case Managers in our Specialty Clinic. Case Management RNs assist Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. The RNs are responsible for providing competent, evidence-based care to patients. The 2-page Resume requirement does not apply to this occupational series. This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems. Major duties include but are not limited too: Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Influences outcomes by collaborating with members of the interdisciplinary team. Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. Addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice. Evaluates care and outcomes to ensure timely and appropriate provision of services. Integrates knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex situations. Validates understanding of the rationale for care processes and decisions. Questions clinical practices for the purpose of providing evidence-based care. Demonstrates the ability to cope with and manage competing priorities. Demonstrates competence in practice and decision-making, deliberate planning, and critical thinking skills. Preferred Experience: Hem/Onc and Case Management/Care Coordination Supervisory Controls: The RN is responsible to the assigned rating and approving supervisors and is accountable to the Associate Director of Patient Care Services (ADPCS). Work Schedule: Monday - Friday 7am - 3:30pm Telework: Not Available Virtual: This is not a virtual position.
