Triage Registered Nurse Job Category: Patient Services RequisitionNumber: TRIAG001120 Full-Time On-site Springfield, MA 01103, USA 10,000 SIGN-ON BONUS Qualifications: Current andunrestricted RN license in Massachusetts Current BLS CPR certificationMinimum of one year of nursing experience, preferably in ER oroutpatient triage setting. Required Skills: Excellent clinicalassessment and critical thinking skills, with a solid understanding ofdisease processes and age-specific development. Demonstrated ability todeliver high-quality nursing care in collaboration with physicians andsupport staff. Culturally sensitive, with a strong interest in servingdiverse patient populations. Ability to establish and maintain effectiveworking relationships with other employees, individuals seekingassistance, representatives of outside agencies, and the general public.Proficient in electronic health record (EHR) systems and familiar withoutpatient and ambulatory care best practices. Excellent verbal,written, and interpersonal communication skills. Flexible, professional,and reliable, with the ability to adapt to varying patient needsCommitted to patient-centered care, customer service, and ongoingprofessional growth Bilingual preferred (Spanish, Russian, orVietnamese). Principal Responsibilities and Duties: Receives andresponds to both telephone and in-person triage requests from patients.Conducts nursing assessments, exercises clinical judgment, answershealth-related questions, provides patient education, and offersguidance following established clinical protocols and guidelines.Documents telephone triage, patient education, and patient instructionaccurately and precisely within the electronic medical record. Schedulessame-day and follow-up appointments with providers to support timely andcoordinated patient care. Provides ongoing patient education, clinicalassessment, and support to patients while working closely with medicalproviders to deliver coordinated, high-quality nursing care. Performsnurse visits as needed, providing clinical assessments, patienteducation, and nursing interventions following established protocols.Administers prescribed medications, vaccines, and treatments followingapproved clinical protocols and standards of nursing practice. Ensuresaccurate and timely documentation of all administrations in theelectronic medical record. Serves as a resource for staff, offeringclinical guidance, nursing support, and educational opportunities asappropriate to promote staff development and quality patient care. Otherduties as assigned. Working Conditions: This position involves prolongeduse of a telephone and extended periods of computer-based work at aworkstation. It requires the ability to sit for long durations, withoccasional standing and walking May be required to lift and carryobjects typically weighing between 5?20 pounds.
