We are missiondriven, aiming always to empower, embrace differences and be BETTERTOGETHER in our approach to service more than 4,000 individualsthroughout Massachusetts, and in areas of Connecticut, New York, RhodeIsland, and Oklahoma. $500 sign on bonus ($250 at the completionof first week of training and $250 after successfully completion offirst 90 days) Position: Rehabilitation Specialist - Full TimeLocation: Springfield, MA Hours: 40 hours per week Rate of Pay: $26.00Per hour Schedule: Monday - Friday 8:00am - 4:00pm Summary: Responsiblefor gathering data, documenting and presenting each member's goals andobjectives in accordance with ISP regulations; coordinating serviceswith case managers and parent/guardians of program member's goals andobjectives, issues, concerns and abilities; and ensuring the provisionof high quality services. The Rehabilitation Specialist is alsoresponsible for overseeing the screening and prioritizing of referrals;documenting member progress; implementing behavior managementstrategies; investigating incidents and all other duties as assigned.Qualifications: High School diploma with 5 years related experience ORAssociate's degree with three years related experience OR Bachelor'sdegree with one year related experience. Knowledge of and respect fordiverse populations. Previous experience working in human servicespreferred. Requirements: 2 years of previous case management or caremanagement experience required. Ability to use computers to includeMicrosoft Word, data entry, email and internet essential. Experience inCredible preferred. Must be organized, detail-oriented, have solid timemanagement skills and be able to work in a fast-paced multi-taskingenvironment. Demonstrated ability to deliver high level of customerservice/satisfaction with both internal and external customers.Excellent communication, interpersonal and listening skills. Must have avalid driver's license, good driving record, use of vehicle andwillingness to transport participants in personal and/or agency vehicle.
