Organization: VPH Admissions/Reception Job Summary: This is a night shift position, working 7pm - 7am; nights will vary with some on weeknights and some weekend nights Either LPC or LCSW Required Performs behavioral health diagnostic and psychosocial assessments utilizing the DSM, implements group, family and individual psychotherapeutic interventions including crisis intervention, develops treatment plans and disposition plans. Maintains contact with family and community agencies and makes referrals. Actively collaborates with team to meet patient care management and system improvement goals. Represents the behavioral health enterprise on institutional and community committees/task forces. Department Summary Our Psychiatric Assessment Service provides care for patients with immediate behavioral health care needs. We offer immediate help in situations including suicide attempt/thoughts of suicide, depression, anxiety, unusual or unrealistic thinking, physical and/or sexual abuse. and drug or alcohol abuse. We accept calls for assessment any time of day or night. . KEY RESPONSIBILITIESu2022 Provides initial assessment and develops plan of care according to TN Department of Mental Health, The Joint Commission, IPFQR, HBIPS, and CMS Best Practice Guidelines and within the scope of their license by the Tennessee Health Related Boards.u2022 Provides targeted evidence based interventions into clinical practice with patients.u2022 Facilitates transitions to appropriate levels of care via in person, telephone or telehealth contact.u2022 Maintains professional documentation which meets department and VUMC standards as it relates to compliance.u2022 Acts as clinical liaison with patient, family, treatment team and community.u2022 Maintains confidentiality and privilege in accordance with TN state codes and statutesu2022 Upholds ethical standards in accordance with relevant professional associationu2019s code of ethicsu2022 Contributes to the organization by actively participating in process improvement initiatives and shared decision making.u2022 Engages in regularly scheduled supervision as required by the Tennessee Health Related Boards.u2022 The responsibilities listed are a general overview of the position and additional duties may be assigned.TECHNICAL CAPABILITIESu2022 Patient Documentation (Intermediate): Demonstrates mastery of patient documentation in practical applications of a difficult nature. Able to train and educate by setting the example, giving technical instruction, providing leadership, and generally raising the level of performance of others while on the job.u2022 Patient Assessment (Intermediate): Demonstrates mastery of patient assessments in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering results without requiring support and instruction from others. Able to train and educate by setting the example, giving instruction, providing leadership, and generally raising the level of performance of others while on the job.u2022 Patient Diagnosis (Intermediate): Demonstrates mastery of patient diagnosis in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering results without requiring support and instruction from others. Able to train and educate by setting the example, giving instruction, providing leadership, and generally raising the level of performance of others while on the job.u2022 Clinical Counseling (Intermediate): Demonstrates mastery of clinical counseling in practical applications of a difficult nature. Able to train and educate by setting the example, giving instruction, providing leadership, and generally raising the level of performance of others while on the job.u2022 Crisis Intervention (Intermediate): Demonstrates mastery of crisis intervention in practical applications of a difficult nature. Able to train and educate by setting the example, giving instruction, providing leadership, and generally raising the level of performance of others while on the job.u2022 Risk Management (Intermediate): Demonstrates full command of risk management in situations of a difficult nature for a major problem. Conducts analysis, identification, planning and control of risk management procedures and practices at the functional unit level that can adversely impact the business. Conducts prevention, contingency planning and preparation of alternatives for normal problems. Displays knowledge of common risk identification methods such as objectives-based, scenario-based, taxonomy-based and common-risk checking.u2022 Peer Leadership (Novice): Establishes personal goals, as a member of a team, which takes the needs of others into consideration.
