Performs other duties as assigned. Monitors tri-weekly Phase II and Phase III cardiac and pulmonary exercise sessions, assessing patient status via telemetry, blood pressure checks, and pulse oximetry. Addresses patient and family concerns, answering questions concerning recovery potential and program protocol. Motivates and guides the rehabilitation process, modifying patient programs according to individual progress or lack thereof. Recognizes and appropriately responds to any changes in patient condition resulting from the disease process and/or induced by exercise. Markets the program and recruits rehab clients in both inpatient and outpatient settings, making referrals as needed to relevant hospital and/or community programs. Performs insurance verifications and schedules patients for the program. Completes medical record and intradepartmental documentation, reconciling financial aspects of the Wellness program, collecting monthly payments and co-pays, and overseeing program charges. Responds to emergency situations promptly and appropriately, recognizing and reporting significant arrhythmia, respiratory distress, and other acute changes, and performing BLS according to American Heart Association guidelines. Assists with the development of cardiac and pulmonary rehab policies and procedures. Develops individualized, result-oriented plans of care in cooperation with clients and referring physicians.
