Job Description

You'll help shape decisions that elevate both patient outcomes and the future of nursing. Job Summary and Qualifications What you will do in this role: + Assists with admission and discharge processes to ensure efficient throughput and high quality, patient-centered care. + Establishes effective communication with physician offices, hospital departments, staff, and rehabilitation services to facilitate a smooth continuum of care for all joint replacement/spinal fusion patients and families. + Coordinates care with case management to anticipate high-risk patient issues and assist in implementing an efficient discharge/rehab plan for joint replacement/spine patients. + Develops education for orthopedic staff including treatment protocols and orthopedic inservices. + Coordinates postoperative follow-up calls to patients and responds to patient calls addressing any questions and concerns. + Assumes responsibility for coordinating components for Disease Specific Certification programs. Works with multidisciplinary team members on performance improvement initiatives. + Coordinates and provides total joint and spinal fusion education classes for patients scheduled for a surgical procedure. Completes nurse leader rounding. + Participates and coordinates (with the marketing department) community orthopedic educational programs presented by physicians. + Works cooperatively and communicates effectively with all team members to ensure coordination of care and documentation of services provided. + Pursues professional growth and development. What qualifications you will need: + Basic Cardiac Life Support must be obtained within 30 days of employment start date + (RN) Registered Nurse + Registered Nurse Diploma + 2 years of acute care/surgical services experience required + Bariatrics/Nurse Navigator experience preferred Benefits Parham Doctors' Hospital, offers a total rewards package that supports the health, life, career and retirement of our colleagues.
