We want you to apply Job Summary and Qualifications This position supports the mission of Presbyterian/St Lukeu2019s Medical Center of optimizing quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. Within scope, the nurse navigator oversees and directs the care of the BMT or Hematology-Oncology patient and is responsible for collaboration with all disciplines to provide well-coordinated patient care. This position functions to navigate blood cancer patients through the continuum of care including diagnosis, treatment, follow-up and long-term care needs. This includes coordination of chemotherapy orders, prescriptions and individualized follow-up. The hematology nurse coordinator demonstrates competence in the ability to triage patients over the phone and in person. The hematology nurse coordinator demonstrates the ability to interpret laboratory and radiologic test results and communicates results to physician as necessary. They provide patient and family education relative to the patientu2019s diagnosis and treatment process. The hematology nurse coordinator demonstrates collaboration, professionalism, and effective communication in all interactions with physicians, the administrative team, PSLMC employees, patients, caregivers and referral sources. The hematology nurse coordinator acts as an ambassador for the bone marrow transplant program when communicating with other healthcare professionals, payors, and the community. When the need arises, will perform other duties as assigned by Lead. Position Requirements: A. Licensure/Certification/Registration: Current licensure in the State of Colorado as a Registered Nurse, or current active multistate Registered Nurse license; BLS u2013 Healthcare provider. OCN or BMT-CN certification within 18 months of hire. B. Education: Graduate of an accredited school of nursing, BSN preferred. C. Experience: Three to five years experience in hematology-oncology or bone marrow transplant care preferred. D. Special Qualifications: Ability to communicate effectively with patients, families, physicians, and other health care team members. Ability to manage a team and work with others within a team to ensure quality patient care. Strong critical thinking skills. Desire to provide compassionate, holistic care.
