Summary The Acute Services Social Worker is a Senior Social Worker assigned to Urgent Care (UC) and the Acute Psychiatric Unit. The incumbent is responsible for providing crisis intervention, psychosocial screening and assessment, treatment and intervention, specialized case management, care coordination, discharge and after-care planning, health education, advanced health care planning, supportive counseling, caregiver support, and administrative and professional duties as assigned. Responsibilities Total Rewards of a Allied Health Professional Major duties include but not limited to: Serves as the facility Safety Planning in the Emergency Department (SPED) Champion and the liaison with Fort McCoy for post-discharge collaboration of wrap around services with Psychological Health Programs (PHP) of active-duty soldiers. Serves as the Salvation Army Voucher Coordinator for the facility. Works independently with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems using his/her educational training, clinical experience, VA policies and accreditation standards in administering direct services. Complete thorough assessments to determine the psychosocial functioning and needs of Veterans and/or their families, caregivers, or survivors. Actively participates as a member of the interdisciplinary treatment team in the development and implementation of treatment goals and interventions as well as collaborating with Veterans and/or their families, caregivers, or survivors. Facilitate community placements through collaboration with Veterans and their families or caregivers as well as interdisciplinary treatment team members to ensure that appropriate community placements are completed in a timely manner. Provide consultation to interdisciplinary treatment team members regarding psychosocial needs of Veterans, their families, caregivers or survivors and the impact of the identified psychosocial problems on the Veteran's health care planning and compliance with treatment. Maintain a level of productivity and quality consistent with the complexity of the assignment and with Social Work, accreditation, and facility standards. Provides mentorship and clinical supervision to enrich social work knowledge and skills to competently provide quality care and improved clinical outcomes. Ensures supervision and consultation sets clear, appropriate, and culturally sensitive boundaries. Participate in late night Urgent Care coverage. Participates in the rotation of on-call social workers, including modification of tour of duty to accommodate Veteran care needs on weekends, if indicated. Perform other duties as assigned. Work Schedule: Monday-Friday; 8:00am to 4:30pm Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
