Summary The Registered Nurse - Inpatient Flow Case Manager functions as a systems coordinator for the plan of care; monitors progress through the expected hospital course and coordinates discharge planning with the patient and their physician team. Responsibilities The RN - Patient Flow Inpatient Case manager delivers and improves holistic care through collaboration strategies with others to provide comprehensive nursing for the complex patients in the assigned area. Major Duties include but are not limited to: Performs an initial and ongoing assessment of patients to identify needs, issues, resources, and care goals, and identifies critical factors for achieving desired outcomes for discharge. Functions as a systems coordinator for the plan of care; monitors progress through the expected hospital course and coordinates discharge planning with the patient and their physician team. Collaborates with the patient and care providers to evaluate and update changes in the management of the patient and the discharge plan. Initiates referrals to other disciplines and/or services as appropriate. Advocates for fiscal responsibility in the management of care through effective utilization of resources. Establishes ongoing relationships with professional/health related groups within the community. Recognizes complex situations that impact patient care and intervenes using sound judgement, professional knowledge and appropriate conduits to resolve issues. Provides support to the in-patient nursing unit(s) by coordinating outpatient home care. All other duties as assigned, pertaining to this occupation, to meet the needs of the Agency, Department, Facility, and/or Service. Requirements Conditions of Employment U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
